Why do most companies suffer from poor hiring success?

Studies demonstrate that in most companies, executive and managerial hiring is not much more accurate than 50%. Now imagine the failure rate increased substantially below the managerial level since the rate of sophistication regarding hiring declines quickly below the executive and senior management level.

Why do companies accept what is essentially random results in hiring? There is not a single process in most companies where 50% success is acceptable. How about the payroll checks you write to your employees? How about the bills you send to your customers? You wouldn’t accept in those processes - so why do you accept it in the hiring process?
